I just got back not long ago from my monthly check-up at klinik ibu mengandung. As i read thru the thread, there's a need to have a referral letter in order to give birth in government hospital. Today i asked the nurse in the klinik ibu mengandung. They said i need not to bring any referral letter, only need to bring the pink colour book since i am with "white sticker" (indicating normal condition).

If i were to give birth in Universiti Hospital, she told me that i can directly go there when i want to give birth, no need to do any pre-registration. Is it true? Further, she said i can continue my check-up at the Klinik ibu mengandung until i give birth.

Is it necessary for me to change my check-up to Universiti Hospital at 7 months? Today i have my first jab of Anti-tetanus and shall have the 2nd jab next month, was thinking that if it's necessary to change my check-up to Universiti Hospital, i will do it next month after my 2nd jab. What's the different between having check-up in klinik ibu mengandung and Universiti Hospital?

I think i heard that we need to pay RM500 for downpayment if we were to give birth in Universiti Hospital. Is it true? I am getting all sort of information which lead me to confusion. Please help.

Ya, it is true. No need referral to go to any gov facilities to deliver. In fact no need to even have antenatal ckeck up or having the pink book. Gov hospital will accept ANY patients (Malaysian or non Malaysian, legal or illegal, afford to pay or unable to pay). The policy of gov hospital is NEVER REJECT PATIENTS. It is better you have the pink book or whatever antenatal record to avoid the doctor to do the clerking and double work for them.

No need to have your check up at university hospital if you are low risk (white label).

In KKIA, the check up done by nurse mainly. In UH, the check up done by doctor mainly. But if you are low risk, it doesn't make a difference. The nurse will sceen you by their protocol. If any risk factors involve, they will refer directly to hospital.

Yes you need the deposit in UH (which is usually higher compare to gov hospital). In gov hospitals, masuk dulu, talk money later.

UH and HUKM are under ministry of education so not really gov hospitals as all gov hospitals are under ministry of health.

Both UH and HUKM are crowded in their public wing. So my suggestion is to do your check up at KKIA and only go to UH when you are in labour. They will take you regardless what they say about booking. No point go for booking if your pregnancy is low risk. I work in these setting before, we will ask you to continue follow up at KKIA if your pregnancy has no problem. So if you go UH, you will probably wait 3-4 hours and ask to go back own KKIA clinic to continue follow up till delivery. Wasting your time and their time. Unless your KKIA doctor say you need specialist attention than it is worth going to have booking at UH.

Sorry, i have lotsa question in mind because this is my first pregnancy and the nurses explanation is very surface. Seems like they didnt expect that there will be complication....but yet, i feel worry because i have never give birth before.

If you reach 40th week and not in labour, you need a referral letter for possible induction. Usually the KKIA will give you the letter without you asking.

At 3rd trimester starting 32nd week, they will see you 2 weekly till 38th week then weekly till labour.

When you are in labour go to the wad bersalin or PAC (patient assessment centre) of the O&G department of the hospital. Every gov hospital has this unit just for pregnant ladies in labour.

SO before you go make up your mind which hospital you want to go and get yourself familiar with the route to that hospital.

Do not want to start a new thread and wish to get some information about "discharge & leakage of amniotic fluid". Basically, how we differentiate whether it's a normal discharge or it's actually amniotic fluid leakage? Since the beginning of my pregnancy, i noticed that i have lotsa discharge...be it thick yellowish discharge or "milky fluid discharge". Recently, i read alot about amniotic bag leakage during 2nd trimester and i am getting worry about it. I do not know how to differentiate whether it's normal discharge or leakage as this is my first pregnancy. Some of my colleague told me that if it's leakage, we can see some blood in the discharge, but i am not too confident on the answer. As i am schedule for check-up in Klinik Ibu Mengandung next month, i do not know who to refer to as i didnt' go for private check-up. Is there any indication to differentiate discharge & leakage of amniotic fluid? i.e..smell, colour..blood or etc.

Since i am getting my monthly check-up in Klinik Ibu Mengandung, the nurses does not do scanning for me. How do i know whether the liquid in my amniotic sack is consider enough for baby? I am reaching 29 weeks this Saturday, and i can feel the kicking of my baby very intensely. It is normal? Cos sometimes, i can feel the pain. Nurses in Klinik Ibu Mengandung told me that they won't do any scanning for me anymore until 40 weeks (provided if my baby still do not want to come out yet).

Is it recommended to do a scanning every month to check whether my baby is ok? Cos until now, i do not know how much does my baby weight. Please help

Leaking means leaking of liquor, from the history usually the patient will say there is a gush of fluid flowing out just like urine. The fluid usually not as thick as the discharge and usually clear to slight yellowish. And it has a distinctive sweet odour. Usually don't have blood. This is different from show which is a mucous like material and usually blood stained. Vaginal discharge usually quite thick and not as much as leaking liquor. Commonest cause is candidiasis, a form of fungal infection in which the discharge is whitish in colour. Typical text book description is whitish or whitish-gray cottage cheese-like discharge, often with a curd-like appearance. When the discharge is greenish or yellowish in colour, this is usually indicating bacterial vaginosis or secodary bacterial infection on top of the candidiasis. Both can easily treat with antifungal and antibiotic.

So in order to diagnose, the doctor will usually do a speculum inspecition. Often vaginal discharge can be diagnosed by visual inspoction. The doctor will also look for pooling of liquor or a gush pf fluid comes out when you are asked to cough, both these indicate leaking liquour. An ultrasound maybe done to measure the liquor volume which may reduced in leaking liquor.

There is a different of management of leaking liquor and vaginal discharge because leaking usually mean there may be preterm labour (hence problem associated with prematurity) and increase risk of intrauterine infection which may affect the outcome of baby. Vaginal discharge usually easily dealt with treatment and usually no problem for the baby. So if you are in doubt I suggest see a doctor at least to have spculum examination to clear your doubt.

Your baby kicking intensely mean your baby is active which is a good sign. Strictly speaking, you don't need an ultrasound every month as the nurse can monitor the baby growth from measuring your uterine height. However ultrasound enable the doctor to keep tract the measurement of parameters so that a growth chart can be plotted and early sign of growth problem can be detected.

Baby weight can be estimated by ultrasound through computer calculation using various parameters OR by experienced hand's palpation.

Thank you for the information. All this while, the discharge is liqiud yellowish in colour. But i did not mentioned much about it during check-up because it happens since pregnancy and they said it's normal for preggie lady to have lotsa discharge.

Normally nurse will only ask me whether I am "sihat" or not. Then will listen to baby heart beat thru the doppler, then the nurse will "picit-picit" my tummy (dunno why but i guess measuring the baby head kuar). sometimes, i doubt whether this is correct measurement or not, because they just put a measurement tape on the part that they "picit" to my private part.

They didnt check my private part either. Then, will continue to prescribe the supplement for me. Sometimes when i urinated, i can feel that there's yellowish discharge, if i wait for a little long while, then there's some liquid coming out also (watery, light yellow) - do not know whether this is the mixture of "urine with discharge" or it's the leakage because it has no "foul smell"...just normal, not stinky. Just now you'd mentioned that the leakage should smell a little bit "sweet". I do not know to determine this either.

Since Klinik Ibu Mengandung is not doing any further scanning, i am just curious how they can ever detect that some "mommy to be" are actually having leakage which harm the baby in the tummy? Do they do anything to detect this earlier?

You did mentioned that yellowish thick discharge means infection, does this infect my baby cos for me, this has happened for quite some time... i am worrying dead. Further, i read fluene post and noticed that cervic dilation happens without mommy actually noticing it. How does this happen? Cos sometimes, i do not know whether baby is getting heavier or what, most of the time when i urinated, i feel like my baby wants to come out dy....

I didnt feel those contraction yet (as mentioned by other members in this forum) but sometimes at night, can feel tummy painful, maybe baby don't like my sleeping position. Hubby said that i'd been thinking too much and read too much of horror pregnancy stories.

I do not know....throughout my pregnancy, i just hope that my baby will be fine. i just want to make sure that everything will be fine as i want to protect my baby. Thanks for explaining as i have no one to turn to.

The nurse's way of measuring the fundal height is correct. SHe will put one end of the measuring tape at your symphisis pubis (private part) and the other end at the highest point of your uterus. The measurement in cm should be the same as the number of your gestational week. (you can look through your pink book, there is a column for this, the number in pregnancy weeks should be about the same as height of uterus in cm). Checking private part is not in their protocol. If you really think this is bothering you, ask to see a doctor.

Difficult to comment from your description. Leaking usually will happen regardless whether you're urinating or not. How they detect leaking? Majority is thorugh patient history which lead to proper exmaination by doctor. There is some methods to detect earlier but is not practical and not recommended in normal antenatal check up.

Usually vaginal infection like candidiasis and 2nd bacterial infection won't affect the baby. Prolonged infection MAY cause preterm labour (theoractically) and ascending infection especially during preterm premeture rupture of membrane.

Fluene has twin pregnancy, the risk of preterm labour is very uch higher than single pregnancy. So no need to worry about this aspect at all. At this point, your uterus is getting larger and inserting pressure on your bladder and pelvis thus you will have this pressure like sensation.

You may experience on and off contraction which is IRREGULAR, these contraction is called Braxton Hicks contraction and usually last for few seconds. It is a normal occurance. Only if you think the contraction is REGULAR and with leaking, you must go to hospital immediately as this may mean preterm labour.

Hi,

I am just wandering whether we can engage or arrange a nurse from Klinik kesihatan to go to our house to check on mummy and baby. Maybe like guide on breastfeed and check on the wound (for mummy) and method of bathing a baby. I know some private hospital do have this facilities, but was thinking if Klinik Kesihatan have this facilities, it should be cheaper lor.

yes, the klinik provides the service. however, if u're in KL u'll need to go to yr nearest KKIA to request it within 7 days of giving birth coz in KL they hv a lot of cases so need a special request before they'll come. i think other towns smaller, so the nurses able to come for each case?

not sure whether they'll check yr wound though, from what i understd they normally check bb? just go to the klinik & ask on yr next checkup.

Yes, I think can cos my SIL who gave birth in private hospital had nurses from Klinik Kesihatan to come check on her & baby. The nurse will check on her stitches & baby's condition. But I am not sure how she registered for it.

Anyone gave birth in selayang hospital? And the gynae is Dr. param?

Dr Param is one of the senior consultants. There are more than 10 specialists there.

Oh good, though give birth at Private Hospital still can arrange the nurse from klinik Kesihatan. Ok, I will check with the Klinik Kesihatan. I am from Selangor, hopefully not so many request as compare to KL.

so long you on regular check up at klinik kesihatan or klinik desa, even you do not deliver bb at gomen's hospital, they will come check to check you provided you inform them when you give birth.

they supposed to come 10 times. but then they onli show up 4 times for my princess and wife due to long weekend on raya and etc.

anyway, they check on bb whether got jaundice or not, etc.

for wife, you can even ask them how to breastfeeding, etc. they show you how.

they are very friendly. for my wife, she was 2 weeks in JB and now back Kluang. i called up Kluang klinik desa, they came to house to check on wife too eventhough we do not check up there.

hubby did inform KKIA that i have give birth to my little princess but they didnt come to visit also.....but just want to ask....my baby is pre-term born at 34weeks, having prolonged jaundice...today already 3 weeks old but the jaundice remain.....the other day went to hospital....level at 191....but it's actually increasing...the trend is like.....247, 267 (admitted for photo), 147 (discharged after photo), 171, 195, 202, 191.........what can i do so that baby jaundice can subside? very "fan"

Just follow your doctor advise and continue the frequent folow up. Preterm babies tend to have immature liver function hence the jaundice. Need some time to normalize. At the same time, monitoring is necessary because too high the bilirubin level can damage the brain.

been in and out hospital and KKIA....baby still having jaundice at day 27. just want to ask you further, if my baby recover, will the level checked drop to 0? or there's an indication saying that baby no longer having jaundice? yesterday went to KKIA and the level increase from 169 to 176.....the level has been increased and decreased since start.

Further, would like to ask....is it premature baby can digest formula milk? my milk production is low and i need to have mix feeding.....the doctor prescribed me metoclopramide & maxalon....but when i check with pharmacy...they said this type of medicine is meant for nausea and vomitting....how true is it? any side effect? should i take or how?please advice

dear nckeat,

been in and out hospital and KKIA....baby still having jaundice at day 27. just want to ask you further, if my baby recover, will the level checked drop to 0? or there's an indication saying that baby no longer having jaundice? yesterday went to KKIA and the level increase from 169 to 176.....the level has been increased and decreased since start.

Further, would like to ask....is it premature baby can digest formula milk? my milk production is low and i need to have mix feeding.....the doctor prescribed me metoclopramide & maxalon....but when i check with pharmacy...they said this type of medicine is meant for nausea and vomitting....how true is it? any side effect? should i take or how?please advice

Maxalon (brand name) is metoclopramide. It's meant to treat gastro problem ie heart burn & etc. The medicine has the side effects of increasing the hormone which produce milk. Motilum (brand name) which contains of Domperidone, another gastro medicine also give the same side effect.

How come your doc prescibe u the medication w/o giving you proper instruction as to how often & quantity?

My gal's jaundice last for 1.5 months before it was gone. My Paed is cool about it since it's below 200. No treatment is required and just continue to breastfeed. BTW my gal was born at Week 37.5

Beside the immature liver function, prolonged jaundice could be due to breast feeding as well (which is benign).

Maxolon is used to promote breast milk production.

The rare side effect is oculogyric crisis, a form of abnormal movement which the symptoms and signs include incredible restlessness, agitation, malaise, or a fixed stare. Then comes the more characteristically described extreme and sustained upward deviation of the eyes. In addition, the eyes may converge, deviate upward and laterally, or deviate downward. The most frequently reported associated findings are backwards and lateral flexion of the neck, widely opened mouth, tongue protrusion, and ocular pain. However it has no long lasting damage.

If possible, do not give mixed feeding because breast milk production need the baby sucking to stimulate the hormonal changes for milk production.

By giving mixed feeding, you reduce this stimulation and hence even less production. Also the artificial rubber nipple with confuse the baby and the baby may eventually choose the artificial nipple rather than natural nipple and thus abandon the breastfeeding completely.

Though most of the causes of jaundice are benign but for prolonged jaundice, it may be best to be referred to a paediatrician for further investigation.<h3 class="r">

I am now 28 weeks pregnant but still not decide whether want to give birth at government hospital or private hospital. My mom ask me to go government, she said safer. Can I know anyone ever gave birth ar HKL or hospital selayang? I heard that HKL got a lot of the training doctors ... tell me if you had info. Thanks :)
