The repair of episiotomy wound is least of the concern. The important thing during delivery is to make sure the complication such as post partum haemorrhage is prevented or properly managed.

Most episiotomy wound is easily sutured even by the most inexperience staff as the skill is easily acquired (see one, assist one and do one).

The complicated one such as those with multiple vaginal tear is the most most difficult to be sutured and often ended with so called ''ugly'' scar.

But vaginal is a very forgiving organ, usually it will heal very well after few weeks.

To prevent lax vaginal, episiotomy suture is not the most important factor. Pelvic exercise can helps. Also limiting your deliveries to 2 or 3 may also help.

Deliver multiple times (e.g. more than 5) is deifinitely the risk factors for lax vaginal.

You don't choose a hospital because of the skill of repairing vaginal tear or episiotomy wound but the ability to handle major complication such as post partum haemorrhage.

f00lgal, KKIA for the monthly checkups or nearing to the end every 2 weeks & every week checkup. ultrasound i went to private clinic, around RM100. basically KKIA just monitors yr pregnancy & give u vitamins.

guess, i had a friend who went to a super-expensive, very experienced (more than 20 yrs), well-known private gynae, she gave birth to her 2nd child & ended up with 2 holes instead of 3 holes in spite of his sewing skills. there was some negligence involved la...some long story but yup, the tear was that bad. she had to go back for surgery twice after that if i'm not mistaken. there is no guarantee on the amount u paid that the gynae will be performing at 110% each time i guess? :P

The worst kind of tear is the 3rd or 4th degree vaginal tear which involve the anal sphicter. This type of tear need specialist or surgeon to repair.

Fortunately it is relatively rare to happen.

But when it happen the best person to repair might be the surgeon.

In private when a surgeon is called mean another few k charged to your bill.

In public hospital, there is always a surgeon oncall 24 hours and it's free with no extra charge.

Anyway the purpose to doing episiotomy cut is to reduce such tear. An artificial tear such as this is a clean cut and easy to repair as compare to the natural tear which may be extended into vaginal and ragged which sometime difficult to repair. But again 4th degree tear is relatively rare and need not unnecessarily worries anyone.

So with this 1 poor girl, there are thousands of girls delivered normally without complications.

I went to KKIA for my 1st check-up today. Previously all my check-ups are done in private hospital. I must admit that they (nurses) are very thorough with their examination and they even showed me how to perform breast self examination. I thought that BSE is not a neccesity when you're pregnant. Truly enlightened!

By the way, the nurses are very friendly and I also got my H1N1 jab for free. Private hospitals are charging a lot for this jab. They also ask me to return next week for my ATT jab (Anti tetanus jab). May I ask why is this jab necessary? Does anyone knows? I have never heard my gyane (private hospital) mentioned of this jab before.

KKIA nurses said they also provide house to house visit before you deliver to check on mother's condition and also after you deliver to check on both mother and baby. I'm very surprised with their standard of service.

Kudos to goverment KKIA. They have truly done a good job.

ATT is given as standard jab for all pregnant ladies in gov facilities. Twice for 1st time mother, once for the rest.

I go for both. i did in Pantai Medical center, Bangsar. For govt i did in Klinik Tanglin beside masjid negara. For my 2nd baby I did in Columbia asia. but for gov i need to go 1st to nearest govt clinics for registration. and after that they transfer me to clinic Tanglin. Clinic Tanglin close to my office.:)

The good thing about govt clinic when i in my kampung, Baling Kedah, private hospital is to far.so this bidan kampung from govt clinic always came to my house and check baby & me. hehe.. no need to go outside during pantang..

i'm 18 weeks pregnant and have been doing my antenatal check up in KKIA, Cheras.

i also did my check up in a private maternity ward but the charges are exorbitant!

imagine rm275 for a blood test and the service is really lousy.

The result came bek saying i don't have enough of blood and had to do another test.. dunno wat it is ler.. not very sure too because they refused to answer much and asked me to go bek to see the doctor.. n yes, the doc also asked us to go for the 4D scan which will add up to another few hundred bucks... so i'm thinking of discontinuing the private visit

I wouldnt mind continuing at KKIA but the place hasn't got any MO for the past 2 mnths

i tot of goin to HUKM for my next antenatal check up...

anyone of you have any idea on how to go about?

I know we need something like referral letter but considering that my KKIA has no MO, do you think the doc at private clinic will be willing to give?

And does HUKM accept early pregnancy?

This is the first time being a mom and both my husband and i are really clueless.

If we have the money, we don't mind goin to private but trust me, the service is worse than the nurses at KKIA. The doctor is nice but the frontrunners-admin staff were rude and stuck up! The first private clinic we went to was even worse.. the nurses are nice but the doctor looked like he left his soul behind! I'm not even thinking abt private hosp due to the price!

Dear all,

i went for my 1st time check up at KKIA Pandamaran this morning...

put aside the waiting period, i would say they did a great job!! i was there for 5 hours for the check up until my HB oso :sauer2:

All of the staffs r friendly & patient despite pack with people, they will attend 2 u in detail explaining wutever is neccesary (though they sounds like reading from a book...perhaps they'd been doing it day in & day out until like robot liao :p), breast checking (which out of my expectation), and blood test on every visit (inclusive of HIV!!!), imiediate result on blood samples (surprisingly!!!), explain the important of breastfeeding....& a lot more...and they performed ultrasound scan for me as well

due to my mum is on diabetic medication, they would like to do OGTT on me..so i will need to go there 7 morning 8 morning again 2ml

i would say the long waiting period could be due to the lousy system whereby they need to repeat filling up my details with pen on photocopy paper (or form 2 b more accurate) for at least 9 times (if not mistaken) in different files  i really dun get the point y need to fill in so many copies...bcoz they shoulb be able to save all these repetitive job by using online system ma...

lastly i think high possibility they r under man power or improper man power management...bcoz 1 staff is engaging with 1 patient for too long period causing the waiting period 2 b long for others...

Dear all,

I have 2 questions, can anyone answer me ?

1. Does gov clinic give antenatal classes

2. Does gov clinic give ultrasound scan

Hi,

Im into my 37th week and have been going to Tanglin Clinic Ibu & Anak for antenatal check ups since week 10 i think. Its gov clinic (polyclinic maybe?). Im not sure if they give antenatal classes but they do ultrasound bout 2 or 3 times during the whole pregnancy. I didnt do scanning though cuz I dont believe in it. Only with doctors though and doctors only in once or twice a week and i think each time only one doc. Normally, the nurses will do check up on you and not doctors. But dont worry as you will meet the doctor especially if you're high risk or problematic.

Oh and each check up was free :) They even give me the vitamins everytime I went there. For free too :))

I also went to govt clinic for my follow up (am from Kuching and there are 3 clinics around and always full with mummies).

My strategy is .. either go after 10.30am or late afternoon like after 3.30pm =P well sometime I still need to wait for an hour alto only few mummies are there for the follow up.

Mmm .. my experiences with them are so far so good. The nurses there are okie at least not rude and they give free supplement like folic acid, iron and multi-vitamin. And I have my few injections there for free too. But the only thing that make me not happy is when I went for my glucose test they make mistake on my reading and I have do it again at my gynae there and the staff at the 'blood unit' aren't that friendly, especially that particular woman (I don't know her name, but I will always remember her face =p) she is rude and blaming when I still wanna do another test at my gynae there since I have done it here then I told her because you can see the reading from these two reports, why have so much difference? then she kept quiet a while and continue said, well but still high .. gess .. I just don't bother to talk to her.

As for the doctor, ya .. only 1 doctor for morning and another 1 for afternoon, so far the doctor is just fine but they never do any scan for me as they knew that I'm having my follow up with my gynae too.

My friend went to the same clinic and I saw the few scans that they done for her, not as clear as mine so I don't bother to have my scan over there.

Hi,

I think u have misunderstood my questions.

What I meant was, is it necessary for preggers to go to KKIA for health records purposes?

Is it necessary for the health ministry to keep records of every malaysian preggers for their own purposes?

And By not attending KKIA will it do any differences?

Actually a lot of ppl are going to both private and government for checkups, not only preggers, but also normal body checkup.

I just want to know by going to gov, besides cost saving, what are the other benefits and importances.

Thank You

No, it is not necessary. In Klang Valley, the KKIA clinics are run by DBKL and hospitals are run by MOH. Both of them don't share record.

KKIA also don't keep records as they will give you an antenatal book (the pink book) to carry around.

They only keep record for post delivery home visiting (for some high risk cases). After that your record will go to archive and will never seen / being retrieved again.

Even for normal body check up, this is only available in certain hospital on request.

The difference of KKIA and private care are:

KKIA

Care by nurses / medical officer

May not have ultrasound

Lots of lectures like breast feeding, postnatal care etc

Community based care

Risk stratification before referral

Free supplement / immunization

Postnatal Home visit (available by request / high risk case)

Zero / low cost

Private care

Care by same specialist

Ultrasound each visit

None, if available usually with cost.

Individual based care

Generally depend on ability of specialist

Supplement / immunization with cost

Postnatal Home Visit not available

High cost

hi,

can i know is there any charges if we request for the post natal home visit service from KKIA?

im currently staying at kota damansara wit hubby, however my parents is staying at ampang. i am wondering which kkia shud i go?

this is bcoz i heard abt the post natal home visit, and i plan to stay ampang for my confinement. am kinda in dilemma which one i shud go now :(

Am not sure what you mean by "actual day for pregnancy checkup" but the KKIA (I'm from Kota Kinabalu) I went to during the last month of my pregnancy opened 7 days a week (half days both on Sat & Sun). The first time I went there was when I was 36 weeks along (I had been going to a private hospital before) and the registration and checkup was about 3 hours. A bit slow because KKIAs are normally short on labor, plus they do everything manually. I was lucky because they weren't many mommies then. The trick is to always go after 11am, or after 3pm (in KK anyway). Followups are normally quicker (about 15 minutes or so where they measure your b/p, fundal height, do an external examination, and urine test). They will also give you free prenatal vits - altho I'd been taking my own since I still had mine from the private hospital). The initial registering/check up should include an Ultrasound scan by a doctor.

If you are unmarried, no problem. Just tell them you are a single mother. They don't require a marriage certificate to register. Just bring along your identity card.

Through experience, I say there really is no difference between the private hospitals and KKIAs. Besides the waiting time, of course. But you can avoid this by going late in the afternoon. The reason I went for my antenatal checkups at the govt clinics was (mostly) to save cost. They do a complete checkup minus ultrasound checks for zero cost. So you can easily save RM60 per visit. Of course, having opted for an elective LSCS, I eventually gave birth at a pvt hospital. However I am now continuing my postnatal and baby checkups at KKIA (they even give free birth control! ;-)

Hi all,

I am currently in my 1st trimester. So far I have just visited a private gynae twice, for scanning and checkup.

Last week, I went to register at KKIA. I am really satisfied with the service and the nurses are very friendly.

When I went into the Dr's room, she asked me if I had seen any other gynaes before coming here. I told her yes and did an ultrasound scanning once. She did ask me if I were going to deliver under that gynae and I honestly told her I haven't decided where as it's still early.

She told me to come back next month for checkup and bring my scan and report(private gynae's) for her review.

My qs is, is it okay to show her my report? If yes, do I always have to bring my gynae's report to show her at every KKIA checkup? I just want to be sure because I am planning to visit both KKIA and private gynae during my pregnancy.

Thank you all.

I went to Taman Datuk Harun Klinik last week, wanted to register for bb injection. Now I am 8 mths. When I reached there, I told the nurse that I want to register just for bb injection and not gynae. She advise me no need to register now, just bring my bb and her birth cert after delivered. Izzit true?

Those are colour coding for your pregnancy risk. Of course, green is low risk and red is high risk. So, either HUKM made a mistake or you have a high risk pregnancy!

hope this help

Yes it's true. After delivery, you just need to go back to the same KK that you follow up for your pregnancy, and they will do the rest. There will be nurses too sometimes that drop by your house to check on you and baby. Just to make sure both of you are well. In KK, they are going to weight baby and vaccinate.

Those services are free!

Check out my site on immunization.
